It does make a difference.

In feudal Japan many Samurai were banned from wearing their swors near their Emperor for fear of assination.

Their close bodyguards would always have the blade sheathed pointing down and away as a sign of peaceful intent and repsect.

The only ever had it pioting up up if they intended to kill somebody with it or as an act of open defiance to their superior.
